Welcome to Sullivan High School Health Center!

 

ACCESS at Sullivan High School is a primary care doctor’s office located inside Sullivan High School. The health center is operated by ACCESS Community Health Network, a not for profit organization health center whose mission is to deliver quality and affordable health care to the Chicago area community.

 

The ACCESS at Sullivan High School Health Center functions independently from, but in close partnership with Sullivan High School. The medical team includes a Nurse Practitioner, Medical Assistant and a Licensed Clinical Social Worker.

 

In addition to providing primary health care services, the health center also offers a variety of educational and support programs. Examples include Pathways to Peace, a therapeutic art therapy program, Expressions of Grief and Loss Support Group for students to cope with the loss of a loved one and the Nutrition Group, a nutritional education program focused on healthy eating, body image, avoiding fad diets, exercise, stress and making better food choices

 

Hours of Operation


The center is open Monday through Friday from 8:00 a.m. until 4:30 p.m. and accepts Medicaid, All Kids, and most private insurance plans. The staff speaks both English and Spanish, and we encourage communication from parents. Any questions should be directed to Mary Crang, Nurse Practitioner/Site Leader at 773-761-5309. 

 

Available Health Care Services


ACCESS at Sullivan High School Health Center offers the following comprehensive services:
  • First aid for minor injuries
  • Diagnosis and treatments of illnesses
  • Immunizations
  • School and sports physicals
  • Laboratory services
  • Reproductive health services (including contraception, pregnancy testing*)
  • Prenatal Care
  • HIV and STD screening
  • Health education & prevention programs
  • Counseling up to five times before parents are notified*
  • Referrals as needed 

 

Guidelines for using the ACCESS at Sullivan High School Health Center


Students seeking to use the center’s services must adhere to the following guidelines:     

  • Students may visit the health center by appointment only. 
  • Appointment times should be scheduled for a student's lunch hour or after school.
  • Students should bring all signed consent forms at time of appointment
  • Students should bring their insurance card on the day of their visit and be prepared to pay any co-payments as required by their insurance plan on the day of service
  • In the event of an emergency, students can use the health center during class time, but only if accompanied by a pass from the classroom teacher.
  • Students without insurance will be charged a fee of $3 per visit. However, the ACCESS at Sullivan High School Health Center will provide comprehensive health care services to students unable to pay this fee. 

 

Required Forms


The following forms are required to allow the health center to provide medical services to Sullivan High School students under the age of 18. Click on the individual form to download and print:
  • Consent To Treat Form (English)
  • Consent to Treat Form (Spanish)
  • All Kids

 

If a parent or guardian chooses not to sign the consent form, the student is still able to receive certain confidential services*, such as counseling and reproductive health care services as permitted by the Illinois State Law. When a child reaches the age of 18, parental consent is no longer required to receive medical care.

Memory Quilt Project

ACCESS at Sullivan High School Health Center recently finished a Memory Quilt project. Sullivan students were invited to creatively “honor” a loved one who is no longer with us. Students are having either black and white or color photographs of a loved one transferred onto a piece of fabric and sewn into a quilt that will be hung at Sullivan High School.
